When estrogen starts declining (and this can begin in your mid-30s with perimenopause), a cascade of changes kicks off that makes weight management significantly harder:

  • Fat redistribution. Estrogen helps keep fat stored in your hips and thighs. As it drops, fat shifts to your midsection. That stubborn belly fat that appeared out of nowhere? Hormones.
  • Muscle loss. Declining estrogen and testosterone accelerate the loss of lean muscle mass, which directly lowers your resting metabolic rate. You're burning fewer calories just existing.
  • Insulin resistance. Hormonal shifts can change how your body processes blood sugar, making you more prone to storing fat.

Here's where it gets exciting. A Mayo Clinic study published in 2025 looked at women taking tirzepatide (the medication behind Mounjaro and Zepbound) and compared outcomes between women who were also on menopause hormone therapy and those who weren't.

The results were striking:

  • Women on hormone therapy lost 35% more weight than those taking tirzepatide alone — 17% total body weight vs. 14%
  • 45% of women on HRT achieved 20%+ weight loss, compared to only 18% without
  • The hormone therapy group also saw better improvements in body composition — meaning they preserved more lean muscle

This isn't anecdotal. This is peer-reviewed, published research showing that hormone optimization and GLP medications work better together than either one alone.
